Application Process

Create Account

Register by completing the Sign-Up Form.
Check your email for the Registration Code.
Submit Application

Login and click the Health Certificate Application tab.
Complete the form and upload the required documents, then click Submit.
Medical Examination

Visit an Accredited Diagnostic Facility or Laboratory to complete the required medical tests.
Payment

Go to the Payment tab and settle the required fees.
Download Health Certificate

After payment validation, your Health Certificate will be available through your dashboard and email address.
